Forms for New Members

 

Please open and fill out the following forms (those that apply to you), and bring them with you for your first class. New CrossFit members should use the Client Profile and Liability Waiver; those who are bringing kids in, use the KidFit form for participants, or the Kids in the Facility form if you are going to have your kids in the kids' room while you work out.
